A CPVC sprinkler adapter fractured after eight years in service at a hotel, operating at slightly elevated water pressure. Examination revealed numerous environmental stress cracking (ESC) sites caused by improper cement joining practices, including excess solvent and air bubbles. FTIR analysis confirmed solvent attack and environmentally induced chemical changes, while the final fracture occurred transversely, likely facilitated by an external side impact near the failure site. PMI Specialists identified installation-related issues and recommended improved joining practices and inspection of similar components to prevent recurrence.
